Program Description: The PSR Program efforts are directed toward increasing the consumer’s level of functioning in personal and social skills, to encourage compliance with medication regimen, and to increase independent living. Community Hospitals, Social Service Organizations and Community Mental Health Programs may make referrals. 

Eligibility Criteria: 18 yrs. or older, previous psychiatric hospitalization with diagnosis, manageable behavior, willingness to participate in group activities.

Services Offered: The PSR Program offers Individual/Group Therapy and Counseling, Psychiatric Consulting, Socio-Cultural Enrichment, Life Skills, Personal Development, Community Inclusion Activities/Events, Case Management Services, Men/Women Issue Focus Groups, Medication and Education Compliance.

Service Area: Boundaries are Roosevelt Road to 75th Street, and Lake Michigan west to the Dan Ryan.

Cost: Should be Medicaid eligible.
