Program
Description: The program provides comprehensive
developmental training services and pre-vocational training to persons
with developmental disabilities, whose condition is attributable to
mental retardation, cerebral palsy, epilepsy, autism and other forms
of developmental disabilities.
Eligible
Criteria: Adults 18 years or older, but not exceeding 64
years of age, have mental retardation as primary diagnosis, medically
stable and able to participate in at least five hours of program
activities daily and reside within the geographical area.
Services
Offered: Person-centered individualized training
designed to foster independence. Community living and Community
Integration skills that include mastery of personal identification
information, survival skills, travel, use of community facilities,
money handling skills, time management skills, socialization,
recreational and out door camping. Pre-academic and academic
skills, pre-vocational skills and limited productive work skills
leading to sheltered employment (productive level of 25% of industrial
norm). Motor skills (fine and gross motor skills including
training in arts, crafts, ceramics, horticulture). Behavioral
management and modification, limited Counseling, Case Management,
Psychological Evaluation , and Transportation.
Service
Area: Service area extends from Roosevelt Road North, to
127th Street South, and Lake Michigan East, to Western Avenue West

No cost to service-recipient; Medicaid Eligible
