Program Description: A Client and Family Support specialist works along with the family to develop a comprehensive plan for and identify and link the client/family to services they may need.

Eligibility Criteria: Persons that are developmentally disabled or a parent, guardian, friend or relative of a person with developmental disabilities with a diagnosis of Mental Retardation, Autism, Cerebral Palsy, or Epilepsy which occurs before age 18.  

Services Offered:  The Client & Family Support Specialists along with the client and/or family will develop a comprehensive service plan for each client and identify and link the client/family to the services they need such as: Community Education and/or Training, Behavioral Intervention Training, Family Training and/or Education, Medication Review, Administration and Wellness Training, Transportation, Genetic Counseling, Life Skills Training, Goal Setting, Crisis Intervention, service coordination between multiple agencies and development of satisfying personal relationships and leisure skills.

Service Area: Service area from Roosevelt Road on the North, to 67th Street on the South.  From the Dan Ryan Expressway on the West and East to the Lake
